“A Comparative study of Ondansetron, Granisetron and Ramosetron for Prevention of Postoperative Nausea and Vomiting in Elective middle Ear Surgeries under General Anaesthesia”

Introduction: Postoperative nausea and vomiting (PONV) remains a significant problem in modern anaesthetic practice, with an incidence of up to 80% in high-risk group patients. (1) .This study was aim to evaluate and compare the efficacy of ondansetron , granisetron and ramosetron as prophylactic antiemetic in Elective middle Ear Surgeries under General Anaesthesia . Methodology : 90 patients of ASA grade I and II in the age group 15-50 years scheduled for surgery under general anaesthesia were studied. Patients were divided into three groups of 30 patients each according to computer generated random allocation. The patients received the following drugs intravenously. Inj. ondansetron 4mg (group A), Inj. granisetron 1mg (group B), Inj.ranmosetron 0.3 mg ( group C). All the drugs were diluted to 5ml volume in normal saline. They will receive either injection ondansetron 4mg Iv or injection Granisetron 1mg IV or injection ramosetron 0.3mg IV 15 minutes before end of surgery for the elective surgical procedure. Postoperatively all episodes of nausea, retching and vomiting were recorded during 1st 72 hours. Efficacy of the drug was evaluated as a complete response – no nausea, retching or vomiting and no need for rescue medication. The observations obtained were tabulated and analysed using one way analysis of variance (ANOVA) or using chi-square test. Results: During 0-6 post-operative hrs the patients in the group B & C are more free from nausea (86.66%) than group A patients (50%) which is statistically highly significant (P<0.001) During 6-24hrs postoperatively the patients in groupB & C are more free from nausea (86.66% & 93.33%) than group A patients (63.33%) which is statistically significant (P<0.05).Incidence of nausea &vomiting in intergroup statistically whithin 24-48 hrs in group B& C more patients were free from nausea (93.33%), while in A group only 90% patients were free from nausea although it was not statistically significant (P>0.05).During 48-72 hrs postoperatively the incidence of mild to severe nausea & vomiting were less in group B & C patients in comparison to group A patients. Conclusion: Based on this study it is concluded that in postoperative period all the three study drugs i.e. ondansetron,granisetron and ramosetron are comparable to each other for prevention of nausea, retching and vomiting . Both ramosetron and granisatron are better than ondansetron in postoperative period. The ramosetron and Granisetron are more safe & effective than ondansetron.
